A mysterious man known as "the King of Death" roams the streets of pre-war Shanghai in this action-packed prequel to Fist of the North Star. Shanghai in the 1930s is a dangerous place. Foreign governments and Chinese factions have carved the city into different quarters, each with its own laws and government, and all rife with corruption. As the major powers vie for political control, the streets are left to local gangs. Life is cheap, and death is always near. In days past, one man walked these mean streets battling the forces of evil. He was known only as Yan Wang, "the King of Death." Few knew who he really was, but many knew that he was a master of Hokuto Shinken - Fist of the North Star - a lethal martial art. He disappeared years ago after dispatching the city's worst gangsters, and is now rumored to be in Japan. Various people have come to Japan in search of him, but who will find him first?

Long before Kenshiro said, "You're already dead," a different Kenshiro walked the violent streets of Shanghai. The city is in constant crisis as two mafia families battle one another for supremacy. In the crossfire, a little girl named Erika is orphaned and soon taken under the wing of martial artist Liu Fei-Yan. Sorrow seems to follow the girl, as every time Fei-Yan attempts to hand her off, her new guardians end up dead. Fei-Yan decides to bring the girl to a man named Guise but instead crosses paths with powerful Kenshiro Kasumi.

Kenshiro Kasumi is the heir of the ancient martial art called Hokuto Shinken. One day Kenshiro's girlfriend Yuki witnesses one of the hits of Taizanji Kenpo (Hokuto Shinken rival school) and she's killed. Kenshiro comes to avenge her and he's framed for the crime.

In the 21st century, a nuclear war turned most of the Earth's surface into a desert wasteland, which also resulted in the contamination of the Earth's water supply. A man named Sanga has built the fortified haven of the "Last Land", where he rules as its dictator by monopolizing the city's uncontaminated water supply. When he learns that the neighboring residents of Freedom Village are trying to dig up a well for their own, Sanga sends his underlings to sabotage their effort. Kenshiro, master of Hokuto Shinken, gets involved in the conflict between the two regions after saving Tobi, an informant hired by Freedom Village.

A Hokuto no Ken sidestory telling the story of Yuria prior to the Hokuto no Ken main story, including the battle between Shin and Juuza.

Rei is a ruthless man who is traveling the world in search of a certain someone... But Rei was not always ruthless an uncompassionate, it's vengeance the one who stripped Rei from his humanity, and he won't be able to become a normal human being until that vengeance is fulfilled. Right now, he can only roam the world as a furious lone wolf, a lone wolf who preys on those who oppose him.

Juuza is a total rebel who obeys and bows to absolutely no one, and with his terrible self taught fighting style, he has absolutely no one to fear. Juuza once gave up on his true love since he realized blood relation made a relationship impossible... But now that she is dead, Juuza has a thing or two to say to the man who was unable to protect her.

He Who Rides With Me is a special 30th anniversary chapter of Hokuto no Ken, set during the timeline gap between Chapters 136 (the end of the Last Nanto General arc) and 137 (the beginning of the Tentei arc). It was originally published as a two-part story in the May and June 2014 issues of Comic Zenon under the title of Hokuto no Ken: Last Piece and was later collected in Volume 11 of the Extreme Edition (kyūkyokuban) of the manga.

A Hokuto no Ken sidestory telling the story of one of the Hokuto brothers and the main antagonist of the parent story, Raoh.

The story is a spin-off prequel to the original Hokuto no Ken. It centers around Garuda who practices the Nanto Shinchou Ken style of martial arts. In the manga he will fight the five Nanto Goshasei members, impress Rihaku and provide a chance for Kenshirou and Yuria to unite.

The anime was composed of 2-minute long, abridged adaptations of select chapters from the first three volumes. They were placed after the main episodes of DD Fist of the North Star season 2. Every volume also contains at least one gaiden (side story). These gaiden are drawn in Yukito's (the elder sister of Yukito Imouto) normal artstyle, and expand on characters who did not receive gaiden during the Hokuto Gaiden period in the late 2000's. The spin-off manga follows the "genius" Amiba, who can learn any fighting style faster than anyone else. After he is defeated by the original series' protagonist Kenshiro and dies, he wakes up in another world and begins a new adventure.
