High-schooler Kii Kogami is stuck in a rut, loathing the monotonous doldrums of his everyday life. If only something exciting were to happen, something magical. As fate would have it, Kii is about to get his wish, but in a way more terrifying than anything he could have imagined. When a little girl clad in gothic lolita attire appears at school and starts to gruesomely bludgeon, dismember and mutilate all who cross her path, while chanting the mantra “Magical Girl” from her corpse-like lips, the school devolves into a state of bloody chaos. Just how will Kii escape from this murderous magical girl? To make matters worse, the magical girl's victims reanimate and join her killer rampage as minions. Kogami Kii's life has been a living nightmare, as Tokyo is devastated by the savage attack of the magical girls. But blood will flow when the magical girls arrive to offer some healing of their own!

Amidst the devastation of a post-magical girl world, Kogami Kii and his companions find themselves crossing paths at the university hospital. Seriously injured, Akuta hatches a plan to eliminate all of the magical girls at once, but it will take a lot of luck and a little time travel. Meanwhile, Kii awakens to find a mysterious letter in his hand and a gift that will shake the world to its core. Blood will be spilled as a final confrontation between Akuta, Kii and Tsukune's alternative personality, Asuka, draws near. Is there any chance of a “happy ending” for Kii and his friends in this hopeless world?

After the showdown on the hospital's rooftop, the world crumbled into nothingness... And then Kagami Kii, Kaede and the others present Tsukune's rooftop death awakened to find themselves in a new world with no Magical Girls, a parallel reality where Tsukune dies in a car accident.
